By completing this form, you confirm that you wish to participate in activities that include cardiovascular, resistance, and flexibility exercises. You acknowledge that your participation in these activities carries a risk of injury. Furthermore, you confirm that you are voluntarily engaging in an appropriate level of exercise that has been recommended to you.
You also confirm that you have either completed a gym induction or do not require one, and that you are responsible for your own safety and able to use all equipment correctly to avoid injury to yourself or others. You agree to return gym equipment to its proper place and to maintain our gym standards. Any injury sustained will be your responsibility, and this agreement releases Abingdon School Enterprises Limited from all liability for injuries that may occur during your use of the equipment. In addition, you understand that gym use is only permitted during public opening times. Abingdon School employees using these facilities also agree to train only when the gym is not timetabled for use, or outside of school hours, and to uphold gym standards by cleaning down equipment and re-racking weights after use. Employees also understand that, unless specifically requested, they will not receive an induction. All information you provide on this form will be kept confidential.
